WHO CAN MAKE A REFERRAL

Usually the majority of referrals come from:

  • Social Workers
  • Health Professionals
  • Schools
  • GP's

However, anyone may refer to Home-Start

with the consent of the parent.

WHAT HAPPENS WHEN A REFERRAL IS MADE?

  • The referral will be allocated to the appropriate Co-ordinator

  • An appointment is made for a Co-ordinator to visit the family in their own home to explain what Home-Start is all about

  • The family agree to accept Home-Start and decide, with the Co-ordinator, whether to have a volunteer, family group, both or none

  • The Co-ordinator will then carefully select a volunteer, a Family Support Worker or arrange a place in the  group

  • The Co-ordinator will introduce the volunteer or the Family Support Worker to the family or alternatively the family will be introduced to the Family Group

  • Regular support is then provided by the volunteer or Family Support Worker

  • Regular reviews are undertaken involving the family, volunteer, referrer and Co-ordinator

  • The family receives support from Home-Start for as long as it is necessary
